NEUROSURGICAL REVIEW: Accuracy of cortical bone trajectory screw placement using patient-specific template guide system - Dr. Matsukawa et. al.

On the July edition of Neurosurgical Review, the paper Accuracy of cortical bone trajectory screw placement using patient-specific template guide system was published. The authors of the paper are Keitaro Matsukawa, Takashi Kaito and Yuichiro Abe.

The study was conducted utilizing Medacta’s MySpine MC – Midline Cortical, a proven technology for accurate cortical screw placement. This publication highlights how MySpine MC can facilitate both minimum invasiveness and strong screw fixation. According to the paper, a patient-specific template guide for cortical screw placement may be a promising solution to improve accuracy and safety. The goal of the publication is to evaluate the accuracy of cortical screw placement using the template guides.
The results show that MySpine MC was accurate enough for clinical application, confirming this technique as a potentially effective solution to achieve both correct screw insertion and a reduction of complications.
The publication reports a significant result: 99.5% of a total of 198 inserted screws are in the safe zone, showing that the perforation rate with MySpine MC is limited whereas it is up to 20% with the standard free hand technique.
According to the study, MySpine MC accuracy is also significantly reproducible among the spine levels, showing a mean deviation between the planned and the actual screw is about 0.6mm and 1.5° in the linear and angular shift, respectively.
These results compare favourably with the values of Robotic and Navigated assisted techniques while requesting no upfront significant capital investment or maintenance costs.

MySpine MC technology has been recently recognized by MedTech Breakthrough as one of this year’s most innovative medical technologies, and this paper reinforces its strength in leading to a safe and accurate screw positioning. The follow-up visit at 6 months confirmed improved clinical scores, lower perceived pain and good patient clinical outcome.
The pre-operative planning is confirmed to be a key tool for bigger and longer screw selection while achieving an optimal screw trajectory in the cortical vertebral bone. A bigger and longer screw can be expected to deliver high pull-out strength and a strong fixation both in caudo-cephalad direction as in rotation.
Surgeons utilizing the MySpine platform benefit from access to a user-friendly navigation system that provides guidance and tools for every stage of the personalized spinal surgery process. This includes a pre-operative planning web portal developed by Medacta’s engineering team. A detailed 3D model of the patient's spine, obtained via a low-dose CT scan that limits radiation exposure, is then used to plan the surgery. The anatomical insights identified foster a deep understanding of each patient case and create an opportunity for complete personalization, utilizing the surgeon’s preferences to plan implant sizing and trajectory. Based on this plan, the MySpine system develops 3D-printed, patient-specific guides that provide high accuracy in screw positioning, creating the finest surgical approach for a true minimally invasive procedure. As an accurate and reliable muscle-sparing approach, MySpine MC is designed to encourage fast patient recovery and is suitable for both inpatient and outpatient surgery settings.
